Who to Call for Help

Call (561) 848-5200 or (888) 848-5200 and ask for the admissions department. Our Admission Coordinators are available 24 hours a day, 7 days a week. They will take care of everything for you.

Admission Process

The admission process begins with a phone call to Hospice made by the patient, a family member, friend, physician or other healthcare professional. The Hospice Admission Coordinator will ask general information including permission to contact the patient's personal physician to make sure the patient is eligible for Hospice services. A time will be arranged for a Hospice staff member to visit with the patient and/or family to explain how Hospice can help. If Hospice is the service you want, Hospice will take care of the rest.

What Does Hospice Provide?

Hospice provides everything needed to keep patients comfortable. Specially trained hospice staff, including physicians, pharmacists, nurses, aides, social workers, pastoral counselors, music therapists and volunteers, are available to make visits, wherever and whenever needed, to ensure patients and family members needs are being met. Hospice also provides and pays for everything related to a patient's life-limiting illness including medications supplies and durable medical equipment.
