Admissions Team

When a patient is admitted to hospice, he or she is given a Team phone number to contact the Hospice Team directly.

Office hours are 8:00am to 5pm, Monday through Friday. After business hours on-call/triage hospice nurses are available by calling the phone number of your Hospice Team, or call (561) 227-5140 or toll-free (888) 848-5200.

On-Call Coverage/Triage

It is the responsibility of the Hospice Team to anticipate problems before they occur. However, because unpredictable situations can develop, on-call services are available twenty-four (24) hours a day, seven (7) days a week, three hundered sixty-five (365) days a year. Nursing staff is on-call for telephone consultation, to offer reassurance, and make needed home visits. Team members are available for telephone guidance and counseling, as well as home visits.
